Position: Home Health Registered Nurse (RN)

Location: Runnemede, NJ

Job Type: Full-Time | Home-Based with Field Visits

Overview: We are seeking a skilled and compassionate Home Health Registered Nurse to join our growing team. This role involves providing skilled clinical care in the home setting, performing OASIS visits, and traveling daily to patient homes within the local area

Qualifications

• Minimum 1 year of skilled home health experience using OASIS documentation (private duty experience does not qualify)
• Confirmed OASIS and skilled care experience is required
• Active Registered Nurse (RN) license in Pennsylvania
• Valid driver’s license and reliable transportation
• Current CPR certification
  
  

Key Responsibilities

• Deliver high-quality, skilled nursing care in patients’ homes per physician orders
• Complete accurate OASIS assessments and ensure clinical documentation meets regulatory standards
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Monitor and document patient conditions and progress, adjusting care as needed
• Support safe and efficient care transitions in coordination with care teams
