Position Description

Schedule:

  • Day Shift: 7a-7p; 3x12s Mon- Fri; no weekends
  • Night Shift: 7p-7a; 3x12s Mon- Fri; no weekends

Locations: PA 19118 (Chestnut Hill neighborhood)

Equipment used: Vents- EVO and Astral, Trach- Bivona- some are cuffed and some are uncuffed, G-tubes and J-tubes

Population: Many are infants- can accept children up to 18 years of age

Summary

The staff nurse provides the medical care and support to children in Intermediate Care Facilities. The nurse is key in educating families on the proper care for their child. The nurse works as an integral part of the interdisciplinary team to ensure optimum quality care for the individuals who reside in the community homes.

Responsibilities

  • Assess, plan and deliver nursing care for a multi-child assignment (usually 2-3 children). Evaluate each child’s response to nursing interventions.
  • Participate in the development of each child’s Individualized Program Plan when requested by the Care Team. Make recommendations to alter plan as needed.
  • Follow each child’s Individualized Program Plan.
  • Make recommendations to and discuss with the Care Team the need to alter nursing care interventions as each child's condition changes.
  • Inform the Care Team of the need to consult with a physician for any reason. After business hours, contact the Administrator On-Call.
  • Inform the Care Team of any concerns observed which might affect the health and safety of any child. After business hours, contact the Administrator On-Call.
  • Incorporate knowledge of infant and child growth and development in the delivery of nursing care.

Physical Requirements

1. Perform routine childcare to children generally weighing between 20 and 85 pounds. Child care includes, but is not limited to, the following activities:

a. Dressing children and performing oral care

b. Lifting children from cribs, high chairs, floor

c. Lowering children into their cribs, high chairs and onto the floor (includes bending or squatting)

d. Carrying children throughout the house

e. Carrying bath water to bedside in small tub

f. Lowering self to floor to play with and render care to children.

2. Lift and carry medical equipment (such as ventilators, compressors, feeding pumps) or other necessary devised which can weigh as much as 45 pounds.

3. Accompany child during transport in Agency designated vehicle, which may involve:

a. Lifting and carrying child

b. Placing child into car seat

c. Carrying and managing medical equipment

Qualifications

  • Minimum of two (1) years’ experience providing pediatric tracheostomy and life support ventilator management
  • Registered Nurse license for the State of Pennsylvania
  • Home care experience, preferred
  • Willingness to respect and support a home environment
  • Current infant/child CPR certification
  • Valid Driver's License, preferred
