Position Description

Position Details:

  • Job Type: Permanent, Full-Time
  • Pay Rate: $100,000 - $120,000 annually
  • Schedule: Monday-Friday, 9:00 AM to 5:00 PM + two weekend days per month
  • Work Style: Onsite position

Position Summary:

As a High Risk Case Manager (Inpatient), you will coordinate care for patients with complex health needs. You will collaborate with a multidisciplinary team to develop and execute care plans that improve patient outcomes, enhance transitions of care, and reduce hospital readmissions. This role is pivotal in ensuring high-quality, cost-effective care for our most vulnerable patient populations.

Key Responsibilities:

  • Care Coordination: Develop, implement, and evaluate comprehensive care plans for high-risk patients, serving as the central point of communication between patients, families, physicians, and community resources.
  • Quality Improvement: Analyze patient data and readmission trends to identify opportunities for intervention and process improvement; present findings to key stakeholders and participate in Transitional Care Committee (TCC) meetings.
  • Patient Advocacy: Act as a strong advocate for patients by identifying and overcoming barriers to care.
  • Clinical Assessment: Conduct patient assessments in various settings as needed, including homes, skilled nursing facilities, or physician offices, to ensure a comprehensive evaluation of patient needs.
  • Education: Provide education to patients and families regarding their condition and care plan, and support health system staff on topics related to community resources and payer issues.

Requirements:

  • Bachelor of Science in Nursing (BSN).
  • Current Registered Nurse (RN) license in the state of Connecticut.
  • Must obtain Case Management certification (ACM or CCM) within one year of hire if not already certified.

Preferred Experience:

  • Strong clinical experience in an acute care setting.
  • Additional experience in home care, long-term care, or case management.
  • Existing certification in Case Management or a clinical specialty.
  • Excellent organizational, communication, and critical thinking skills.
