Adult Day - RN - Registered Nurse

The Adult Day Care Center located on our Bronx, NY campus is currently seeking a full time Registered Nurse to join our team! Responsible for providing and coordinating multidisciplinary health services for older adults in the Day Care Center, Works with the physician and other team members in implementation of plan of care. Position is Monday - Friday, 10:00AM-6:00PM with alternating weekend shift (8:00AM - 6:00PM) responsibility.

• Admits patients in the program as needed and in a timely manner.
• Participates in meetings to report adherence to frequency and duration of services as per physicians’ order. Reports patients’ progress to team.
• Acts as a coordinator of the multidisciplinary team providing care to patients.
• Establishes a plan of care
• Documents content and process of all services provided.
• Monitors the effect of the care plans on the patient health status
• Appraises care provided, revises goals and interventions to patients’ care as needed.
• Educates the older adults and family regarding the disease process, self-care techniques and prevention strategies.
• Initiates and maintains written and verbal communications to assure optimal patient care.
• Performs UAS Assessment and ReAssessments
• Performs other relevant duties as needed.

Requirements:

• License and current registration to practice as a Registered Professional Nurse in New York State.
• Graduate of an accredited school of nursing
• A minimum of one year experience in acute and sub-acute care and or minimum of one year experience in an Adult Care Center preferred.
• Bilingual (Spanish /English), required
• UAS certified, a plus

Salary $105,000-115,000 & Benefits including PTO, Medical/Dental
