The Home Health Hospice RN is responsible for providing skilled nursing care to patients in their homes, including medication management, treatment planning, patient counseling, and thorough clinical documentation. This role supports patient comfort, adherence to treatment plans, and overall quality of care while maintaining compliance with infection control and clinical protocols.

Key Responsibilities:

• Manage and administer prescribed medications, monitor for side effects, and provide patient education.
• Perform comprehensive patient assessments and document findings accurately.
• Counsel patients and families on care plans, procedures, and treatment expectations.
• Develop and implement individualized treatment plans based on patient needs.
• Ensure compliance with infection control protocols and safety standards.
• Utilize clinical equipment and technology as required for patient care.
• Adapt to changing patient needs, schedules, and environmental conditions.

Required Skills & Competencies:

• Strong adaptability and problem-solving skills in home health settings.
• Knowledge of infection control procedures and home health nursing standards.
• Proficiency with clinical equipment and electronic documentation.
• Ability to work independently and manage time efficiently.

Qualifications:

• Active California Registered Nurse (RN) License
• Current BLS certification (AHA required)
• 1+ year of professional nursing experience; home health experience preferred

Job Type: 2 months Contract

Schedule: Monday – Friday | 8:30 AM – 5:00 PM | 40 hrs/week

Expected hours: 40 hours in a week

Local Pay range: $50-$55/hour

*Offered pay rate will be based on education, experience, and healthcare credentials.

Travel Gross Pay: $2400 - $2600/Week
