Job Summary And Qualifications

What you will do in this role:

  • The Hematology Nurse Coordinator (HNC) functions as a multidisciplinary team member and oversees and directs the care of the complex blood cancer patient.
  • Functions as an advocate and educator for patients from point of entry, diagnosis, and treatment through survivorship.
  • The primary function is to establish a relationship with patients, family/caregivers, and physicians and facilitate effective communication.
  • Coordinate the care plan, including appointments, patient and family/caregiver education, provision and/or enablement of support services, and follow-up.
  • Serves as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, referring providers, supportive care members, e.g., social workers, hospital staff, and the broader healthcare community.
  • Provides a single point of contact and continuity in care throughout the care continuum
  • Assume responsibility and accountability for managing resources to achieve efficient, high-quality outcomes (e.g. Oncology Alert Program) for each complex blood cancer patient.
  • Provides support for interdisciplinary and cross-facility collaboration, e.g., tumor boards, weekly intake meetings, and referring provider communication. Utilizes expertise in complex blood cancers to provide patient and caregiver support and education throughout the continuum of care
  • Participates in the care coordination between the inpatient and outpatient setting and provider visits, as appropriate. Identifies and reduces gaps in care and aids with care transitions.
  • Provide leadership for the Blood Cancer Program team in developing quality, services, and growth. Works with the comprehensive team to capture/monitor data and attain program goals.
  • Utilizes experience in clinical trials to support advanced research endeavors.

What qualifications you will need:

  • ADN required
  • BSN preferred
  • 3+ years of applicable experience
  • BLS (American Heart Association or American Red Cross)
  • Oncology Certified Nurse (OCN) preferred - 1 year grace period
  • Bone Marrow Transplant Certified (BMT-CN) preferred - 1 year grace period
