RN Case Manager

StartDate: 9/1/2025 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in an inpatient rehab setting

MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN license; BLS; 2 years recent acute care case management experience

PREFERRED QUALIFICATIONS – Inpatient Rehab Or Skilled Nursing Experience

LENGTH OF ASSIGNMENT – 8 weeks

SHIFT / HOURS PER WEEK – 8a-5p M-F (36hrs/wk guaranteed)

START DATE – ASAP
