Position Description
Job Summary:
The Registered Nurse (RN) provides skilled nursing care to patients of all ages in their place of residence. This role involves coordinating care with the interdisciplinary team, patient/family, and referring agencies. The RN is responsible for developing and evaluating individualized care plans, ensuring continuity and quality of care, and educating patients and families on health management.
Essential Duties & Responsibilities:
Assess home health and hospice patients to identify physical, psychosocial, and environmental needs.
Develop, implement, and evaluate individualized Plans of Care (POC).
Document all patient interactions, changes in condition, and care plans accurately and timely.
Provide care using infection control measures and OSHA standards.
Educate patients and families on medications, diet, safety, and self-care practices.
Supervise and guide HHA/STNA/LVN staff to ensure quality service delivery.
Assess and manage pain using appropriate techniques and education.
Coordinate referrals to appropriate agencies and monitor third-party payer compliance.
Perform on-call duties and respond to patient needs outside regular hours.
Communicate effectively with patients, families, physicians, and team members.
Attend interdisciplinary team meetings and contribute to care planning.
Required Qualifications:
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (BSN preferred)
Current state RN license
Current CPR certification
Valid driver’s license and reliable transportation with insurance
1–2 years of clinical nursing experience
2+ years of home health experience
Experience with Homecare Homebase software and OASIS documentation
Preferred Qualifications:
CHPN (Certified Hospice and Palliative Nurse) certification
Skills & Competencies:
Strong assessment and critical thinking skills
Effective communication and interpersonal abilities
Knowledge of medications, venipuncture, IV care, wound care, and durable medical equipment
Ability to work independently and as part of a team
Organizational skills and ability to prioritize under pressure

*Offered pay rate will be based on education, experience, and healthcare credentials.
