Immediate Opportunity – PD Night Shift

Riverdale, New York

RiverSpring Living is seeking a Licensed Practical Nurse (LPN) to join our dynamic team of professionals at our Nursing Department at the Hebrew Home at Riverdale!

The Hebrew Home at Riverdale by RiverSpring Living, a world-renowned long-term care organization located in Riverdale—North Bronx, serves over 18,000 people every day, both in the greater New York community and on our beautiful 32-acre campus overlooking the Hudson River. RiverSpring Living provides a full range of care solutions, including nursing home care, managed long-term care, assisted living programs, senior housing, rehabilitation services, and specialized services such as elder abuse prevention and memory care.

The licensed practical nurse will observe, record, and report resident data according to policy and procedure. Maintains a safe and therapeutic environment. Ensure that the appropriate documentation concerning unauthorized discharges is entered in the resident’s medical record in accordance with established procedures.

Per Diem Night Shift Positions Available

Night Shift hours: 11:00pm - 7:00 am, including weekends and weekdays.

Key Responsibilities

Supervisory

  • This position has supervisory responsibilities, which include planning, assigning, scheduling, and reviewing work assignments. The LPN supervises the GCT and CNA, who report directly to the LPN.

Operations

  • Observes, records, and reports resident data according to policy and procedure.
  • Administers resident care as assigned by the registered nurse within the Nursing Department guidelines.
  • Participates in giving direct resident care.
  • Assumes the responsibilities under the direction of a registered nurse associated with the role of a charge nurse or associated primary nurse, as the neighborhood position requires.
  • Directs and supervises non-professional staff as assigned.
  • Follows all personnel policies and procedures (i.e., attendance, dress code) of the facility.
  • Ensures that appropriate documentation concerning unauthorized discharges is entered in the resident’s medical record in accordance with established procedures.
  • Performs routine charting duties as required and in accordance with our established charting and documentation policies and procedures.
  • Orders prescribed medications, supplies and equipment as necessary, and in accordance with facility-established policies
  • Notifies the nurse manager/charge nurse of all drug and narcotic discrepancies noted.
  • Develops work assignments and/or assists in completing and performing such tasks.
  • Ensures that department personnel, residents, and visitors follow the department’s established policies and procedures at all times.
  • Documents resident care provided following the nursing process and as per the facility and Nursing Department’s documentation policy and procedures.

Clinical

  • Participates in the formulation, implementation, and evaluation of resident care plans to meet the particular needs of the geriatric resident under the supervision of the registered nurse.
  • Administers medications and performs nursing procedures and treatments according to policy and procedure.
  • Reviews medication administration records for completeness of information, accuracy in the transcription of the physician’s order and adherence to stop order policies
  • Receives telephone orders from physicians and enters the same on the EMR.
  • Signs off physician’s orders on the EMR and transcribes orders to resident’s care plans as required and as per policy and procedure.
  • Prepares and administers medications as ordered by the physician.
  • Ensures that prescribed medication is administered following the 5 rights in medication administration.
  • Ensures that narcotic records are accurate
  • Reviews medication orders for completion to assure that medications are being administered as ordered and treatments are provided as scheduled

Residents and Relations & Support

  • Cooperates with other resident services departments when coordinating nursing services to ensure that the resident’s total regimen of care is maintained.
  • Monitors nursing care to assure that all residents are treated fairly and with kindness, dignity, and respect.
  • Ensures a safe and healthy living environment for all residents.
  • Greets newly admitted residents upon admission and/or escorts residents to the room as necessary
  • Undertakes resident/family teaching for self-care and continuity of care.

Qualifications/Skills

  • Graduate from an accredited LPN program.
  • Valid New York State License as a Licensed Practical Nurse (LPN).
  • Experience in a hospital/long-term care setting.
  • Strong customer service, organizational, communication, and writing skills are essential.
  • Must be competent with Microsoft Office and electronic medical records (EMR).

Compensation

  • Sign-on Bonus of $5,000 ($2,500 at end of Six (6) Months and $2,500 on the one (1) year Anniversary, less applicable taxes)
  • $35.9850 hourly
  • This position is part of the 1199 SEIU Bargaining Unit and is eligible for 1199 SEIU Benefits & Pension
