EFFICIENCY OF PRESTON MEMORIAL HOSPITAL

POSITION SUMMARY

The LEAD LPN-Mountaineer ChalleNGe Academy is a unique position within the Mon Health System. The LEAD LPN is a standalone role resident at Camp Dawson in Kingwood, WV. Camp Dawson, a West Virginia Army National Guard facility in Preston County, WV, a dedicated military post operated by the West Virginia National Guard, housing the WV Army National Guard's officer candidate school, a Delta Force training base, and Mountaineer ChalleNGe Academy designed to give academically challenged teens a second chance at obtaining their basic education.

In addition to providing LPN support, the incumbent will be responsible for the coordination of, efficiency, and cost of healthcare services on behalf of its patient population. The incumbent will be responsible for researching quality payment programs across the Hospital’s payer network and collaborating with the Hospital’s quality and process improvement teams to maximize the quality and financial benefit to both the patient population and the Hospital.

The position is a self-managed role, requiring 24-hour coverage and availability to cadets and military staff at Camp Dawson site.

EXPERIENCE

  • One year’s experience in urgent care, emergency department, or medical office setting is preferred.
  • Effective interpersonal communication skills in order to effectively collaborate with physicians and instruct patients and their families.

EDUCATIONAL REQUIREMENTS

  • Highschool diploma
  • Graduate of an approved program of practical nursing
  • Nursing degree, or degree in epidemiology, public health, or related health field
  • BLS Certification

LICENSURE AND CERTIFICATIONS

  • Current West Virginia State Nursing license (Required)
  • Must possess BLS Certification within orientation period.
  • For individuals acting as Employee Health Nurse, the following will be required: COHC, Certified Occupational Hearing Conservationist

ESSENTIAL PILLAR DUTIES

The following are the general expected duties of this position and are not meant to be all encompassing. Additional duties may be assigned as appropriate.

People and Culture

  • Answer cadet, military leadership and parent calls; and determine how to assist them 24 hours.
  • Provide basic patient care or treatments, evaluate nursing intervention outcomes, conferring with other healthcare team members as necessary.
  • Provides follow-up patient care by communicating test results to patients and relaying provider plan of care.
  • Identifies health education needs of patients; develops and implements specific teaching plan appropriate to age and/or developmental stage.
  • Use the computer and telephone to communicate effectively and accurately document medical and demographic information.
  • Correlate and manage large groups of patients/cadets and provide care prior to provider initiated exams.
  • Act as a staff development liaison between site resources, clinic management, and quality to ensure fully functioning operations, capabilities, troubleshooting, limitations, etc.

Integration and Efficiency

  • Assists in the development of departmental strategic plans and documentation policies and procedures, and integrated clinical and financial projects and systems.
  • Facilitates multi-disciplinary collaboration.
  • Identifies and takes proactive action for ongoing problem solving in varies area and departments.
  • Schedules appointments and ancillary services for related and follow-up patient care.
  • Distributes sample medications to patients under the direction of the provider, explaining risks, benefits, side effects, and instructions.
  • Follows up on ordered tests by obtaining results and relaying them to the provider.
  • Follows up with appointments from outside the organization.
  • Collects and records patient history from visits with both internal and external offices and prepares chart prep for next office visit. This is to include patient allergies, medication list, patient and family medical/surgical history.
  • Collects, records, and maintains patient information and vitals for current office visits.
  • Conveys information to provider; obtains and reviews orders; seeks provider input as needed.
  • Develops and maintains constructive and cooperative working relationships with others and maintains them over time.
  • Collaborates with healthcare professionals within the Mon Health System and externally to plan or provide treatment.
  • Analyzes quantitative data to determine effectiveness of treatments or therapies.
  • Maintains medical facility records.
  • Develops operational standards and procedures for the work unit or department.
  • Makes process improvement recommendations to improve operational efficiencies and cost savings.
  • Assists healthcare practitioners during examinations or treatments.

Financial Vitality

  • Maintains inventory of medical supplies or equipment. Order medical supplies or equipment.
  • Contacts insurance companies as needed for prior approval for procedures, tests, immunizations, etc.

Quality and Safety

  • Manages referrals and outside provider visit for large portion patients/cadets.
  • Manages and assembles patients/cadet carts and provides them to MCA administration for proper billing and filing.
  • Requests, obtains, packages medication independently.
  • Properly collects and delivers specimens to hospital laboratory for testing.
  • Manages stock of drug samples, assuming accountability for documentation, records, and inventory.
  • Performs procedures such as injections, CLIA waived testing, and venipuncture.
  • Follows proper protocol for all medical tests, procedures, and use of equipment.
  • Provides a safe environment for the patient/cadet to ensure they have the appropriate colored band per MCA policy.
  • Manages medication and provides education to the legal guardian prior to leaving for holidays.
  • Protects patient rights to privacy and confidentiality.

Growth and Development

  • Demonstrates ability to understand growth and development stages and apply them in practice.
