- Serve as the primary RN case manager for hospice patients, ensuring care aligns with Elara Caring’s mission and vision
- Assess patients and families to determine physical, emotional, psychosocial, and environmental needs
- Document findings and care plans accurately in clinical records and interdisciplinary reports
- Communicate significant changes or concerns to supervisors, physicians, and facility staff
- Guide and educate patients, caregivers, and facility staff on hospice philosophy, pain control, and symptom management
- Provide emotional and practical support during end-of-life transitions, including bereavement assessments
- Collaborate with the Interdisciplinary Team to deliver the Right Care, at the Right Time, in the Right Place
- Graduate of an accredited nursing program (Diploma, Associate, or Bachelor’s)
- Current RN license in the state of practice
- At least 1 year of clinical nursing experience (hospice or home health preferred)
- Willingness to travel up to 50%
- Physical ability to lift 50–100 lbs. and perform patient care tasks
- Valid driver’s license, proof of insurance, and reliable transportation
