Job Details

• Deliver patient-centered care using nursing process in a home health environment
• Assess physical, psychosocial, and educational needs of patients and caregivers
• Develop and update individualized care plans with the patient and care team
• Collaborate with interdisciplinary teams to evaluate and ensure continuity of care
• Educate patients and families to promote understanding and safe home management
• Participate in quality improvement, professional growth, and team communication
• Apply evidence-based practice and contribute ideas from clinical research
  
  

Skills You Need

• Active RN license in North Carolina
• One year of experience in Med Surg, ICU, or ED setting
• Strong critical thinking and assessment skills
• Ability to work independently and prioritize care in home-based environments
