Salary $115,000 - $130,000 per year

Hospice Case Manager for either Brooklyn, Queens, Manhattan, or Bronx

Job Details
Manage caseload of (4) terminally ill Patients per day, generate and follow plan of care to provide patients with Maximum level of comfort and quality of life in home, Skill nursing facilities and In-patient units settings Coordinate care efforts among LPN, HHA and care givers.
Educate Patients, Families and Care givers on medical choices and methodologies Assess patients weekly and as needed to identify necessity for plan of care revisions and seek Interdisciplinary expertise to coordinate highest level of care Gather information on patients and families psychosocial and spiritual unit for symptoms management
Assess terminally ill patients in crisis for need of Inpatient unit Transfer for Symptoms management Maintain up to date computerized patient records Communicate with relevant Physician, Staff members and other agencies Assess patients for recertification eligibility, recertify patients or Plan safe discharge planning.
Work Schedule Monday thru Friday 830 am to 5 pm
The schedule includes working one weekend per month
