Nurse Navigator - Stroke

Full Time - 40 hours/week

Every third Saturday commitment

Newark, DE

The Nurse Navigator is to anticipate, coordinate, communicate, and monitor the patient plan of care through an interdisciplinary team approach with ChristianaCare's stroke patient population.  To ensure desired outcomes with focus on length of stay for an assignment of patients.  Develop action plan upon admission and key performance indicator (KPI) goals are met prior to discharge while facilitating a safe, timely and efficient transition to the next level of care. The nurse navigator is responsible for the goal attainment process and is accountable for identifying and removing barriers that will prevent the patient from reaching these goals.

Education and Experience Requirements:

• Bachelor's degree in nursing required.
• Three years appropriate clinical experience required.
• Prior experience working with stroke patient population strongly preferred.
• BLS certification required.
• Obtain stroke certification within two years from hire date.
• Delaware RN licensure or eligibility for licensure in the state of Delaware
