HealthCare Support is actively seeking a Field Case Manager / Registered Nurse to fill an opening with a first class healthcare company delivering exceptional care in Minneapolis, MN and surrounding areas.

Daily Responsibilities for Field Case Manager / Registered Nurse:

• Conduct home visits, assess needs and develop service delivery plan in compliance with guidelines.
• Managing and staying current to assigned caseload.
• Shift: Monday – Friday, 8 a.m. – 4 p.m
  
  
  

Qualifications for Field Case Manager / Registered Nurse:

• 1+ years of experience as an RN
• Prior experience in home health or field-based role
• Have a drivers license and a clean driving record
