Position Description
Job Overview – Registered Nurse (Community Health):
Compensation: $56/hour
Location: Bronx, NY
Schedule: Tuesday to Saturday, 8:30 AM to 5:00 PM
Deliver person-centered care as a Registered Nurse (Community Health) with our client in Bronx, NY. In this field-based role, you’ll complete UAS-NY assessments, provide skilled nursing care, and coordinate services across home, clinic, and facility settings for PACE program members. Collaborate with interdisciplinary teams to ensure quality outcomes and support medically complex, aging populations. Ideal for RNs with home health or acute care experience.
Responsibilities as the Registered Nurse (Community Health):
- Comprehensive Assessment: Conduct in-home evaluations using UAS-NY and develop individualized care plans in collaboration with the interdisciplinary team.
- Skilled Nursing & Coordination: Provide clinical care, administer medications, and coordinate with providers to support seamless transitions across care settings.
- Patient Education: Counsel patients and caregivers on treatment plans, medication safety, nutrition, and chronic condition management.
- Compliance & Documentation: Maintain accurate, timely records to meet regulatory standards and confirm program eligibility.
- Collaboration & Safety: Partner with Care Managers and supervise support staff to promote a safe, person-centered care environment.
Qualifications for the Registered Nurse (Community Health):
- Education: Degree from an NLN-accredited School of Nursing is required (BSN preferred).
- Licensure: Current and unrestricted Registered Nurse license in New York State is required.
- Experience: 1 year of clinical nursing experience required, preferably in home health (CHHA), acute care, medical-surgical, or critical care settings.
- Technical Skills: Proficiency in UAS-NY assessments and EMR documentation strongly preferred.
- Skills & Attributes: Strong clinical judgment, organizational skills, and commitment to community-based care with the ability to manage multiple priorities independently.
- Additional: Must have a valid driver’s license and meet physical and health clearance standards per NYS and organizational guidelines.
