Senior UAS Assessor (Full-Time & Part-Time)

Bronx, NY | Field-Based

Healthcare Support is seeking Full-Time and Part-Time Senior UAS Assessors to join our care management team in the Bronx, NY. In this field-based role, you’ll conduct initial assessments, create care plans, and directly impact the health and well-being of members in your community.

We handle scheduling and logistics so you can focus on delivering exceptional care.

Compensation

Full-Time: $39.66 – $71.44/hour

Part-Time: Up to $56.41/hour

Schedule

Full-Time: Mon–Fri, 8:30 AM – 5:00 PM (37.5 hrs/week)

Part-Time: 3 days/week – Mon–Wed or Wed–Fri (22.5 hrs/week)

✅ No weekends, holidays, or night shifts

Key Responsibilities

• Conduct UAS-NY assessments for new and existing members
• Develop and update individualized care/service plans
• Identify and coordinate services, specialists, and resources
• Monitor progress and adjust plans as needed
• Educate members/families on services and benefits
• Document all care activities per state, federal, and company guidelines
• Work independently in the field, making critical clinical decisions

✅ Qualifications

• Active, clear NY State RN license
• 2+ years UAS, home health, or homecare experience (OASIS, CHHA, or MLTC considered)
• Reliable transportation (public transit, personal vehicle, or ride services)
