RN Case Manager

StartDate: 9/8/2025 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in an Inpatient Rehab (with possible float to LTACH)

MINIMUM REQUIRED QUALIFICATIONS – 1+ year recent acute hospital case management experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – M-F & at least 2 weekend days per month (Saturday or Sunday)

SYSTEMS – Linq, Prodoc & Protouch

START DATE – ASAP
