Local LVN - Hospice San Antonio, TX

Shift: 8 AM - 8 PM and/or 8 PM - 8 AM

Facility: HOS - SAT - San Antonio TX

Scheduled Hours : PRN Pay Rate $40/hr + $0.70 mileage

Coverage area: Bexar County.

Hospice and/or Home Health experience required & HomeCare HomeBase EMR experience required

Any overtime must be pre-approved and requires written approval from Administrator

Key Responsibilities

• Provide compassionate, direct care to hospice patients and their families
• Perform assessments and update the Plan of Care
• Collaborate with the interdisciplinary team (LPNs, CNAs, and support staff)
• Maintain timely and accurate documentation
• Educate families on care procedures and hospice philosophy
• Participate in on-call rotation as needed
• Manage symptoms and provide end-of-life support
• Ensure compliance with infection control and quality standards
  
  

Client Requires

• Current LVN license with 2+ yrs experience
• HCHB experience
• Hospice and/or Home Health exp.
• MMR, Varicella , Hep B, TDAP
• Valid/current TB skin test
• Seasonal Flu (by 10/01/2025)
