Position Description

Compass Healthcare Consulting and Placement is conducting a search for an experienced Home Care RN, Registered Nurse, for a Director of Patient Services DPS opportunity for a Home Care Agency servicing New Jersey. Qualified candidates will have prior Home Care knowledge. Experience as an RN Director, Clinical RN Manager or Field RN Supervisor as well as previous Acute Care Nursing experience, is preferred. The position requires some travel completing in-services with RNs making patient visits within New Jersey. Valid driver’s license and reliable transportation is required.

The RN Director of Patient Services DPS will oversee RNs providing professional nursing services to patients at home while remaining in compliance with all State and Federal rules and regulations while adhering to all company policy, procedure and philosophies. They will also act as a resource to staff, collaborate with leadership on Quality Assurance, Performance Improvement and other projects, conduct documentation review, supervision, training and support of staff.

Responsibilities:

  • Supervises Field RNs who perform initial patient assessment on patients admitted to the agency for care.
  • Coordinate and Communicates with the patient's physician in determining the need for home care and in developing, implementing and revising the Plan of Care.
  • Communicate with physicians as necessary.
  • Develop an individualized goal outcome Plan of Care.
  • Supervise care according to the Plan of Care.
  • Ensure that physician’s orders are obtained initially, reviewed as required and communicated appropriately.
  • Assist with coordination of homemaker and HHA supervision.
  • Assists and Develops the Aide Plan of Care reflecting initial patient assessment and Plan of Care.
  • Assess the appropriateness/effectiveness of all paraprofessional caregivers on a regular basis and annually.
  • Submit and maintain accurate and timely documentation to client care.
  • Document supervisory visits.
  • Review paraprofessional documentation.
  • Conduct training programs for PCA’s and HHA’s.
  • Participate in orientation to policies and procedures for staff members.
  • Perform initial and annual competency testing on paraprofessional staff.
  • Act as a mentor when appropriate for other health care workers.
  • Participate in quality assurance and performance improvement activities as assigned.
  • Monitor patient/employee infections – Maintains Infection Control Log.
  • Monitor reporting of complaints and occurrences related to clinical issues. Maintains Complaint/Occurrence Book.
  • Perform quarterly chart audits as designated in regional quality assurance structure.
  • Review and evaluate employee health information as required by the Department of Health.
  • Ensure all HIPAA requirements are adhered to.
  • Will be available for on-call as needed. Follow all Agency policies and procedures.
  • Perform other duties as assigned.

Qualifications:

  • NJ RN with a minimum of a Bachelor’s Degree in Nursing.
  • Valid driver’s license issued by the state in which they work, a satisfactory driving record and reliable transportation.
  • Minimum of one year of experience in medical surgical nursing hospital based preferred. One additional year of home-health based experience is required for conducting applicable training classes.
  • Must maintain all state and federal compliance requirements for licensure and employment.
  • Knowledge of regulatory requirements to Joint Commission/CHAP/DOH b) Fundamental knowledge of the basic principles of nursing and the ability to apply these principles in a home care setting
  • Ability to accurately assess patient health care status and psycho social needs
  • Ability to establish a Plan of Care which will assist the patient to reach desirable health status
  • Ability to function with minimal supervision
  • Bilingual (Spanish or Chinese languages, preferred)
