Job Title: Clinical Manager (RN )Home Health - (Pennsylvania License Required)

Location: Philadelphia, PA (On-site, In-Office)

Job Type: Full-Time/Day shifts- mon-fri

Pay- up to $120K annually DOE

Position Summary

We are seeking an experienced and compassionate Registered Nurse (RN) to join our leadership team as a Clinical Manager.

Key Responsibilities

Clinical Leadership

Support and guide clinical teams to ensure delivery of high-quality patient care.

Develop, maintain, and implement policies and procedures aligned with CMS, Medicare, and accreditation standards (ACHC, CHAP, Joint Commission).

Regulatory Compliance & Quality Assurance

Oversee clinical audits, licensing reviews, and survey readiness.

Ensure compliance with all state, federal, and accrediting body regulations.

Lead corrective action planning and performance improvement initiatives based on HHCAHPS and clinical KPIs.

OASIS & PDGM Oversight

Monitor OASIS documentation for accuracy and compliance.

Implement effective PDGM strategies to optimize patient outcomes and reimbursement.

Collaborate with leadership, clinical, and QA teams to ensure documentation integrity.

Team Development & Recruitment

Qualifications

Active RN license in Pennsylvania (Required)

BSN or MSN preferred

Minimum 5 years of experience in a Medicare-certified home health agency

Proven expertise in PDGM, OASIS, CMS regulations, and home health operations

Familiarity with ACHC, CHAP, or Joint Commission accreditation standards

Experience with EMR systems (Alora experience a plus)
