What You Will Do:

• Provide nursing case management services for individuals with behavioral health needs, ensuring continuity of care.
• Conduct health assessments and develop individualized care plans.
• Collaborate with interdisciplinary team members to coordinate treatment and support services.
• Monitor patient progress, adjust care plans as necessary, and ensure documentation is accurate and timely.
• Educate patients and families about treatment plans, medications, and self-care strategies.
• Facilitate referrals and linkages to community resources.
  
  
  

Who You Are:

• Registered Nurse with 1+ years of experience licensed in Massachusetts (required)
• Strong assessment, documentation, and communication skills.
• Reliable transportation and willingness to travel within the service area.
