Home Care, Behavioral Health RN Case Manager

How You’ll Make An Impact

• Assess patients and families to identify behavioral, physical, psychosocial, and environmental needs
• Develop, implement, and evaluate individualized plans of care
• Conduct initial and follow-up nursing evaluations, adjusting care as needed
• Communicate significant health changes with supervisors, physicians, facilities, and care teams
• Provide education and guidance to patients, families, and facility staff on behavioral health and symptom management
• Supervise, support, and document care provided by behavioral health aides and LPNs
  
  

What We’re Looking For

• Current, unrestricted RN license
• At least 1 year of clinical nursing experience
• Willingness to travel within the branch/office coverage area (up to 50%)
• Reliable transportation, valid driver’s license, and current auto insurance
• Associate’s or Bachelor’s Degree in Nursing (preferred)
• Home Health or Hospice experience (preferred)
• Experience with Homecare HomeBase (preferred)
