RN Complex Care Manager
- Monday - Friday
- Outpatient/Ambulatory setting
- Seeking an RN with 3 or more years related clinical experience in Ambulatory, Outpatient Clinic, Community Health, Home Health, or related
- Bilingual Spanish Required
- Active MA LPN or RN License
- Certified Case Manager (CCM) preferred
