A Brief Overview
The Registered Nurse (RN) in our high-volume outpatient Primary Care clinic serves as a critical care coordinator and patient advocate within a hospital-owned physician practice. This full-time role focuses on patient care coordination, clinical triage, and telehealth services rather than hands-on procedures. The RN acts as a liaison between patients and the medical team, ensuring continuity of care and effective communication across the care continuum.

What You Will Do
- Serve as the clinical point of contact, coordinating care plans in collaboration with physicians.
- Perform prompt telephone triage and manage incoming patient calls.
- Provide nursing support via telehealth platforms, including video visits.
- Manage walk-in patient triage and facilitate urgent care needs.
- Coordinate transitional care management to reduce hospital readmissions.
- Handle referrals, follow-ups, and pre-authorizations for tests or specialists.
- Manage medication-related tasks, including prescription refills and prior authorizations.
- Educate patients on chronic disease management and health maintenance.
- Document patient interactions accurately in the Electronic Health Record (EHR).
- Adhere to standardized triage protocols and clinical algorithms.
- Collaborate effectively with multidisciplinary teams for coordinated patient care.
- Advocate for patient needs and provide empathetic customer service.
- Participate in quality assurance initiatives and clinic meetings.
- Graduate of an accredited school of nursing required
- Bachelor's Degree BSN preferred
- Minimum 1-2 years of nursing experience, preferably outpatient or primary care

- EHR documentation and patient data management.
- Telephone and clinical triage proficiency.
- Familiarity with telehealth platforms.
- Competency in care coordination workflows and referral management.
- Knowledge of healthcare regulatory standards and quality metrics.
- Exceptional verbal and written communication.
- Compassion and empathy in patient interactions.
- Strong teamwork and collaborative skills.
- Excellent organizational and multitasking abilities.
- Critical thinking and problem-solving capabilities.
- Adaptability to dynamic clinical environments.
- Professionalism, confidentiality, and patient advocacy.
- RN - Registered Nurse - State Licensure and/or Compact State Licensure Current NJ License upon hire required
- BCLS - Basic Life Support BLS through American Heart Association upon hire required
- National certification related to specialty area upon hire preferred
- ACLS - Advanced Cardiac Life Support May be required dependent on specialty area
- PALS - Pediatric Advanced Life Support May be required dependent on specialty area
- NRP - Neonatal Resuscitation May be required dependent on specialty area
