Title: Registered Nurse - SOS Program
Location: New York City, NY (Onsite)
Employment Type: Full Time, Day Shift, Mon-Fri 09:00 AM - 05:00 PM
Compensation
Pay Range: $64,400-$92,000 Per Year
Description
Engage with individuals experiencing homelessness using the Critical Time Intervention model.
Facilitate connections to housing, social services, and community supports.
Participate in outreach efforts both above and below ground.
Collaborate with community partners to address safety and service disconnection issues.
Key Responsibilities
Conduct health assessments for program members.
Provide community-based nursing support without prescribing medications.
Assist members with navigating housing, food stamps, and program enrollment.
Participate in canvassing efforts to engage individuals in need.
Coordinate mental health and housing supports.
Evaluate the need for emergency room or hospital involvement when necessary.
Qualifications
RN license (new graduates welcome to apply).
Interest in community-based, street outreach nursing.
Ability to engage vulnerable populations with empathy and professionalism.
