Pride Health is seeking a Licensed Practical Nurse (LPN) to support our client’s medical facility based in Brooklyn, NY – 11235. This is a 3-month assignment with the possibility of contract-to-hire, offering a great opportunity to join a top-tier healthcare organization.

Job Title: Licensed Practical Nurse
Location: Brooklyn, NY – 11203.
Pay Range: $29.00/hr to $33.75/hr
Shift: 5*8's. 3 Shifts timings available (11:00 PM to 7:15 PM OR 3:00 PM to 11:15 PM OR 7 AM to 3:15 PM)
Duration: 03 Months (Contract) with a possible extension.
*Pay rate is based on years of experience and educational qualifications.
Key Responsibilities:
Patient Care & Clinical Responsibilities
- Provide direct nursing care to patients in accordance with established nursing standards and protocols.
- Accurately assess and evaluate patients' conditions using available diagnostic results and clinical observations.
- Implement individualized care plans based on assessments and revise them as patient conditions change.
- Administer medications and treatments as prescribed by physicians or as per protocols.
- Monitor and document patient responses to medications and treatments.
- Perform routine nursing procedures (e.g., wound care, vital signs, catheter care, etc.).
- Ensure patient safety, comfort, and dignity at all times.
Documentation & Evaluation
- Complete thorough and timely documentation of patient care, interventions, and outcomes in accordance with department policies.
- Conduct and document reevaluations at defined intervals and during any significant change in patient condition.
- Maintain accurate medical records, ensuring compliance with HIPAA and institutional policies.
Care Planning & Coordination
- Participate in the development of individualized patient care plans with measurable goals.
- Collaborate with RNs, physicians, and other healthcare team members in planning and revising care.
- Identify discharge needs early and coordinate resources for post-discharge care in collaboration with the Care Manager and interdisciplinary team.
- Monitor and adjust care to align with patient progress and condition changes.
Patient & Family Communication
- Educate patients and families about health conditions, care procedures, and post-treatment instructions.
- Address patient and family concerns professionally, ensuring their understanding of the care process.
Teamwork & Supervision
- Assist in the supervision and delegation of tasks to non-professional staff, such as CNAs.
- Communicate effectively with the healthcare team to ensure continuity and quality of care.
Compliance & Professionalism
- Adhere to the philosophy, standards, and policies of the Nursing Department.
- Follow infection control and safety protocols in all nursing activities.
- Maintain current knowledge and skills relevant to the role through continuing education.
Skills and Competencies:
- Ability to work in an environment that may include exposure to dust, noise, and other moderate discomforts without undue difficulty.
- Ability to:
- Hear, speak clearly, and communicate effectively with patients, families, and staff.
- Walk, stand, bend, stretch, and perform other physical movements required for patient care.
- Sit for extended periods to complete documentation and administrative tasks.
- Stand for long durations at the patient bedside and in procedural areas.
- Read, write, and interpret documents accurately.
- May be exposed to various occupational hazards, including but not limited to:
- Mechanical, electrical, radiant, explosive, burn, and chemical hazards.
- Potential exposure to blood, body fluids, and tissue during routine duties.
- Hazardous waste and materials requiring identification, handling, and proper disposal.
- Must be able to:
- Handle and assist in lifting or transferring patients to/from exam beds and procedural stations.
- Operate and clean clinical or diagnostic equipment as required.
- Demonstrate knowledge and compliance with all Kingsbrook/Rutland policies and procedures related to hazardous waste and hazardous materials.
- Maintain qualifications and certifications applicable to job-specific safety and hazard protocols.
- Understand and adhere to all emergency and safety codes (e.g., fire, disaster, chemical spill, etc.).
Education and Certifications:
Education:
- Graduation from an accredited school of practical nursing; or
- Completion of an approved program in professional nursing.
Licenses & Certifications:
- Current and valid Licensed Practical Nurse (LPN) license issued by the New York State (NYS) Department of Education.
- BLS (Basic Life Support) from AHA (American Heart Association)
