Home Health Registered Nurse

HealthCare Support is seeking an experienced Home Health RN to join a mission-driven healthcare provider recognized for delivering compassionate, community-based care. We currently have openings in Bronx/Westchester and Bronx/Upper Manhattan. This field-based role offers the stability and work-life balance you’ve been looking for—Monday through Friday, daytime hours—while giving you the opportunity to make a direct impact in the lives of patients in your assigned community.

Schedule: Monday – Friday, 8:30 a.m. – 5:30 p.m. | No nights, weekends or holidays required!

Pay: $120,000/yr – $125,000/yr (DOE)

Daily Responsibilities For Home Health RN

• Perform home visits to deliver wound care, medication administration, and health assessments
• Provide nursing care in alignment with individualized care plans, physician orders, and best practices
• Assess and monitor patient conditions, promptly communicating updates to care teams and physicians
• Educate patients and caregivers on condition management, wound care, medication adherence, and home safety
• Collaborate with interdisciplinary teams to address patient needs and coordinate services
• Document all care and outcomes accurately in the electronic health record system
  
  

Required Qualifications For Home Health RN

• Current, clear RN license in New York State
• Degree from an NLN accredited school of nursing
• 3+ years of clinical nursing experience, including 1–2+ years in a Certified Home Health Agency or Long-Term Home Health Care Program
