Field Case Manager – Registered Nurse

HealthCare Support is seeking a Field Case Manager – Registered Nurse in Minneapolis and surrounding areas to work with a statewide community dedicated to helping people with disabilities and older adults thrive in their homes.

Pay Details: $34/hr. – $43/hr.

Shift Details: Monday – Friday, 8:00 AM – 4:00 PM I NO On-Call!

Daily Responsibilities For Field Case Manager – Registered Nurse

• 3-4 visits per day – assessing clients, creating care plans, and setting up home and community-based direct care services for clients with complex health needs as determined by their assessment
  
  

Required Qualifications For Field Case Manager – Registered Nurse

• Current unencumbered license as a Registered Nurse in the state of Minnesota
• 1+ year of experience as a Registered Nurse
• Prior experience in home health or field-based role
• Have a valid driver’s license, clean driving record, and reliable transportation to use for work driving purposes
