Title: Registered Nurse - ACT Team

Location: Manhattan, NY (Onsite)

Employment Type: Full Time, Day Shift, Mon-Fri 09:00am-05:00pm

Compensation

Pay Range: $64,400-$92,000 Per Year

Description

Provide comprehensive nursing care and support to program participants.

Engage in health assessments, treatment planning, and medical service coordination.

Work collaboratively with the ACT team to ensure integrated care delivery.

Lead group sessions on health, wellness, and recovery topics.

Educate staff on medical care and medication procedures.

Advocate for participants to access necessary medical care and community resources.

Key Responsibilities

Conduct nursing assessments and develop individualized care plans.

Implement and evaluate nursing care strategies.

Coordinate health services with the ACT team and other healthcare providers.

Supervise LPNs on medication management and administration.

Ensure participants have access to primary care providers and regular medical exams.

Administer medications, vaccines, and perform basic medical procedures.

Maintain accurate health records and documentation.

Qualifications

Licensed and registered as a professional nurse in NYS.

BSN preferred.

Valid driver's license required.

Bilingual (Spanish) is desirable.

Experience with serious mental illness, forensic populations, co-occurring disorders, or HIV/AIDS preferred.
