Job Details

Be the clinical anchor for patients facing serious illness. As the Palliative RN Case Manager, you lead comprehensive, person-centered care—identifying physical, psychological, and social needs; coordinating the right interventions; and keeping patients and families informed and supported throughout the journey.

What You’ll Do

  • Deliver direct patient care within the State Nurse Practice Act and organizational policies.
  • Own a manageable caseload anticipate risks, de-escalate issues early, and prevent avoidable utilization.
  • Assess thoroughly and often complete initial and ongoing assessments covering diagnosis, treatment plan, functional status, acuity, and other pertinent factors; perform medication reconciliation at each visit as appropriate.
  • Drive the plan of care develop, update, and revise plans in collaboration with physicians/practitioners and the interdisciplinary team; clearly communicate changes to patients, families/caregivers, and team members.
  • Coordinate services end-to-end initiate timely referrals (e.g., PT, OT, MSW, ST, Dietitian, DME, and specialized programs) and report needs to clinical leadership and/or practitioners.
  • Monitor clinical changes re-evaluate status, consult with clinical leadership/practitioners, and ensure risk adjustment (HCC) and CMS 5-Star measures are addressed when applicable.
  • Educate patients and families teach diagnosis-specific self-management in clear, practical terms.
  • Lead serious-illness conversations address POLST and DPOA and conduct the Serious Illness Discussion; revisit as conditions evolve.
  • Document with discipline produce accurate, timely notes aligned with the plan of care and regulatory standards.
  • Engage as a team player participate in IDG/IDT meetings, in-services, and the onboarding of new team members.
  • Support coverage needs participate in holiday coverage as assigned.
  • Model professional standards be accountable for outcomes; communicate proactively; maintain a constructive, solutions-focused approach; make sure patients and colleagues know how to reach you when issues arise.

Qualifications

  • Current, unencumbered California RN license.
  • 1+ year of professional RN experience within the last 3 years; palliative care, hospice or home health experience preferred.
  • Solid knowledge of primary medical and nursing care principles in serious-illness/palliative settings.
  • BLS certification (required within 60 days of hire).
  • Strong listening, communication, and multitasking skills.
  • Proven ability to collaborate within an interdisciplinary team.
  • Valid California driver’s license, proof of insurance, and willingness to travel.
