Job Details

This role offers the opportunity to build strong connections with patients and families while ensuring their physical, emotional, social, and spiritual needs are met through a comprehensive plan of care. You’ll coordinate closely with physicians, social workers, chaplains, aides, and volunteers to deliver truly holistic hospice care.

What You’ll Do

  • Serve as the primary nurse for a caseload of hospice patients, providing skilled nursing care guided by the plan of care.
  • Conduct thorough assessments and update care plans as patient needs change.
  • Educate patients, families, and caregivers about symptom management, medications, and available resources.
  • Coordinate services across the care team—including physicians, social workers, chaplains, aides, volunteers, and contracted providers (PT, OT, Speech, Dietary, Pharmacy).
  • Ensure appropriate use and documentation of medications, durable medical equipment, and supplies.
  • Complete all clinical documentation accurately and on time.
  • Participate in interdisciplinary team meetings and collaborate with leadership.
  • Provide on-site evaluations of aides and volunteers to maintain care quality.
  • Participate in the on-call rotation, including some weekends and holidays.
  • Support quality improvement efforts, orient new team members, and mentor nursing students when assigned.

What We’re Looking For

  • Current California RN license.
  • At least 1 year of professional nursing experience (BSN may substitute for 1 year of required experience). Hospice or home health experience strongly preferred.
  • Skilled in primary medical and nursing care, with strong critical thinking and problem-solving ability.
  • Compassionate communicator, able to work with patients, families, and team members during difficult times.
  • BLS certification (within 60 days of hire).
  • Valid California driver’s license and proof of auto insurance.
