Hospice RN Case Manager / Registered Nurse

HealthCare Support is actively seeking a Hospice RN Case Manager / Registered Nurse to fill an opening with a prestigious Hospice organization in Tennessee. Location – Dickson/West Nashville area.

Daily Responsibilities for Hospice RN Case Manager / Registered Nurse:

• This RN Case Manager will conduct home visits or facility visits to provide patient care.
• The RN Case manager will work alongside an NP, RN team, Social worker, and other case managers to collaborate and promote ongoing care
• Provide wound care, monitored vitals, administered medications, and performed death pronouncements as needed.

Schedule: Monday - Friday, 8:00am - 5:00pm

• On-call one night per week from 5pm-11pm (no weekends)
  
  

Required Qualifications for Hospice RN Case Manager / Registered Nurse:

• Active RN License in TN
• At least 2 years of RN experience
• Hospice/Palliative care experience
• Related experience in Hospice or Home Health or Oncology or ICU or Med/Surg or LTC/SNF

Pay Details: $74,173 - $84,886 per year (based on experience) + bonus eligibility
