Schedule/ Shift: Monday – Friday, 8:00 AM – 4:00 PM or 9:00 AM – 5:00 PM

Job Description

The Clinical Assessment RN will conduct high-quality, in-person clinical assessments for members within assigned Boston territories. This role plays a key part in identifying unmet health needs, collaborating on care plans, and supporting regulatory requirements using clinical judgment, electronic documentation systems, and assessment tools.

Responsibilities/ Job Duties:

• Conduct in-person or virtual (telehealth) clinical assessments within assigned territory.
• Perform screenings such as PHQ-9, EASI, Audit, DAST as clinically appropriate.
• Reconcile medications, monitor vital signs, and educate members on chronic/acute conditions.
• Escalate urgent issues to supervisors as needed.
• Accurately document assessments using electronic health records and care management platforms.
• Coordinate with care teams to develop individualized care plans.
• Refer members to services such as behavioral health and long-term care when appropriate.
• Participate in departmental quality initiatives and ongoing clinical education.
• Manage a personal schedule of assessments and track completion.
• Provide assessments via telehealth when appropriate.

Minimum Education and Experience Qualification Requirements:

• Associate Degree in Nursing (required)
• Active Registered Nurse (RN) license in Massachusetts, in good standing
• Minimum of 1 year home care experience (including clinical rotations)
• Experience with MDS (Minimum Data Set) is preferred
• OASIS experience is preferred
• Bachelor’s Degree in Nursing preferred
• Bilingual in Spanish is preferred
