Position Overview

S:US is seeking a LPN for their adult family shelter in Manhattan.

This position is located at E. 31st Street New York, NY 10016

Schedule: Tuesday-Saturday 9am-5pm (one late shift 11am-7pm)

The LPN m eets with the Program Director, Director of Social Services, Registered Nurse, and Social Service staff to coordinate shelter residents to receive ongoing treatment in the community.

The LPN will;

• Take vital signs, such as blood pressure, temperature and weight
• Assist with diabetes medication and treatment
• Take medical histories and enter information in computer systems
• Coordinate with Case Managers, Senior Case Managers and Director of Social Services to ensure proper care, nutrition and treatment plans are followed
• Follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed
• Have flexibility to work a schedule that includes evenings and weekends
  
  

Qualifications

• Completion of accredited program and licensed in New Y ork as an LPN.
  
  

Preferred Qualifications & Skills

• Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences. Bilingual preferred.
