RN Case Manager

StartDate: 9/22/2025 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Discharge Planning Care Coordination / Continuum of Care / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – VA/compact RN license; BLS; 1+ year recent acute hospital case management experience; Discharge planning experience

PREFERRED QUALIFICATIONS – 5 years' experience in specialty; Successful completion of a travel assignment

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:30a-4:30p with rotating weekends (40hrs/wk)

SYSTEMS – Meditech, Careport

START DATE – 9/15/2025
