Title: On-Call Registered Nurse, RN Case Manager

Job Type: Permanent / Direct Hire

Setting: Hospice

Location: Deerfield, IL

Schedule: Full time, 7 days on/7 off, 5P-9A

Pay: $85,000-$98,000 and benefits (flexible scheduling, mileage reimbursement, medical, dental, PTO)

The On-Call Registered Nurse, RN Case Manager duties to include but not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides
  
  

Requirements

• At least 1 year of nursing experience in a related area: hospice, home health or critical care, ICU/Intensive Care
• Associates degree in nursing
• Active and clean Illinois Registered Nurse license
• Hospice experience preferred
• Valid Driver's license and reliable transportation
