Position Description

Care Manager (RN) – Deltona, FL

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Contract Assignment - 3 months ++

Position Overview

The Care Manager (CM) plays a critical role in improving patient outcomes by reducing hospital admissions, readmissions, and emergency room visits. Serving as a resource, advocate, and educator, the CM guides patients with complex health conditions through the healthcare system, fostering autonomy and optimal quality of life. This position collaborates with a multidisciplinary team to ensure coordinated, comprehensive care delivery.

Team Structure & Culture

You’ll join a tight-knit clinical team composed of:

1 Medical Doctor (MD)

1 Advanced Practice Registered Nurse (APRN)

2 Medical Assistants (MAs)

2 Front Desk staff

1 Care Coordinator

1 Care Manager

The team values proactive communication, cross-disciplinary collaboration, and patient-centered care.

Core Responsibilities

Prioritize patient care needs during initial visits; proactively address emerging health concerns

Meet with patients, caregivers, and families to review and personalize care plans under the Transitions of Care program

Track high-risk patients across care settings including acute, custodial, and ambulatory environments

Apply evidence-based disease management protocols to improve clinical outcomes

Facilitate smooth transitions post-discharge including follow-up appointments, home health, and social support services

Maintain clear, consistent communication with care teams, patients, and external facilities

Evaluate cost impacts of care decisions using CM metrics and health plan tools

Participate in daily huddles and weekly Patient Care Coordination (PCC) meetings

Prepare timely, accurate reports for internal tracking and care planning

⚕️ Ideal Background & Qualifications

Required:

Active Registered Nurse (RN) license

Current Basic Life Support (BLS) certification

3+ years clinical experience in primary care, urgent care, or hospital settings

Proven ability to multitask and work independently in a fast-paced environment

Strong customer service mindset and communication skills

Proficiency in Microsoft Office (including Excel)

Familiarity with medical terminology, equipment, HIPAA guidelines

Willingness to complete color blindness test
