Job Description

Hospice RN Case Manager

Location: Macomb County

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Position Type: Full-Time | Field-Based

As a Hospice Registered Nurse Case Manager, you will play a central role in helping patients and families find peace and support at home during life’s most meaningful journey.

We are expanding our team in Macomb County and seeking a compassionate Registered Nurse who is ready to make a real difference.

Key Responsibilities

  • Deliver skilled nursing care in patients’ homes with compassion and professionalism
  • Conduct initial and ongoing assessments, developing tailored care plans for each patient
  • Monitor changes in condition and adjust care as needed
  • Collaborate closely with physicians, social workers, and the interdisciplinary team
  • Educate families on disease progression, symptom management, and comfort care
  • Provide emotional support to patients and loved ones, helping them navigate end-of-life care
  • Ensure documentation accuracy and compliance with all care standards

Qualifications

  • Graduate of an accredited nursing program (Diploma, ASN, or BSN)
  • Current RN license in Michigan
  • At least 1 year of nursing experience (hospice or home health strongly preferred)
  • Reliable transportation and ability to travel throughout Macomb County
  • Strong clinical judgment, communication, and organizational skills
  • Physical ability to perform home visits, including lifting up to 50 lbs
