What You Will Do

• Deliver comprehensive nursing care to program participants in a community setting.
• Conduct health assessments and develop individualized treatment plans.
• Administer medications, vaccines, and provide basic medical care.
• Coordinate care with the ACT team and external healthcare providers.
• Lead wellness groups and educate clients on recovery and health topics.
• Supervise LPNs in medication administration and clinical procedures.
• Advocate for participant access to medical services and community supports.
• Maintain accurate health records and ensure compliance with documentation standards.
  
  

What You Bring:

• Current NYS Registered Nurse (RN) license (required).
• BSN preferred.
• Valid driver’s license (required).
• Bilingual in Spanish is a plus!
• Experience with serious mental illness, forensic populations, co-occurring disorders, or HIV/AIDS is a bonus.
