We are seeking an experienced, dynamic, and compassionate RN Case Manager to join our Home Health team in a hybrid role. The successful candidate will provide top-quality care to patients in the comfort of their homes while also managing case coordination and communication. This position offers the opportunity to work both remotely and in-person, allowing for a versatile work environment. If you are passionate about patient-centered care, have a knack for problem-solving, and have a strong background in home health, this could be the perfect opportunity for you.

Responsibilities

  • Conduct initial assessments of new patients, develop personalized care plans, and coordinate home healthcare services.
  • Regularly review and update patient care plans, ensuring they meet the evolving needs of the patients.
  • Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless delivery of care.
  • Manage the admission, discharge, and follow-up process for patients, ensuring a smooth transition to and from home care.
  • Oversee the completion of all necessary documentation, including the 485, to ensure compliance with federal, state, and agency regulations.
  • Provide skilled nursing care to patients in their homes, including administering medications, wound care, and other treatments as prescribed.
  • Educate patients and their families on disease management, self-care, and home safety measures.
  • Act as a liaison between patients, families, and healthcare professionals, ensuring clear communication and understanding of treatment plans.
  • Participate in regular team meetings to discuss patient progress, challenges, and updates.

Qualifications

  • Must hold a current, unencumbered Registered Nurse (RN) license in the state of practice.
  • A minimum of 1 years of experience as an RN
