Job Summary: Provides direct nursing care to hospice patients, including needs assessment and developing, implementing, and evaluating a plan of nursing care. Provides direct patient care on an ongoing basis in accordance with the Nurse Practice Act and Hospice standards of care. Cultural Behaviors Measurement

Medical Record Audit
Specifications:
1. Education: High School Diploma or GED is required. Associate's degree in nursing.
2. Experience: Two years nursing experience. Hospice or home health experience and CHPN certification preferred.
3. Certification/License/Registration: Must be licensed as a Registered Nurse in the state of Georgia.
4. Position Titles and Number Supervised: None.
5. Special Training/Skills: Ability to work well within an interdisciplinary team setting as well as independently.
6. Physical Demands: Must be able to use visual near and distance acuity. Must have verbal ability to communicate in person and over the phone or radio. Must be able to assist co-workers in transfers and transport supplies, patients/residents, or equipment using good body mechanics. Must have functional range of motion of the cervical, thoracic and lumbar spines, upper and lower extremities with a grip strength of 60-80 pounds, specific to the job class evaluation. Must be able to forward reach, overhead reach, bend, squat, kneel and change from one position to the other rapidly. Must be able to lift 25-60 lbs. and carry 25-50 pounds specific to job class evaluation. Must be able to tolerate high noise, busy atmospheres and noxious odors. Must be able to work in noisy environments and be self directed to complete jobs as ordered. Must be able to remain calm under stressful situations. Must be able to assess and respond to multiple situations simultaneously. Must be able to assist co-workers in multiple overlapping tasks simultaneously. Must be able to prioritize job tasks and organize work space to be efficient and conducive to completing work tasks as assigned in a timely manner. If job class is identified as falling under POPE C, the individual must pass the cardiac recovery test.
7. Mandatory Training Required: Must attend mandatory hospital and departmental annual training.

9. Working Conditions: Works in hospital and clinical environments. May work in extreme heat with hot materials. Must be able to utilize appropriate universal and specific precautions for protection from potential exposure to infectious or communicable diseases.
10. Other (Operation Excellence): Employee must be aware of and fully support the system's policy in Operation Excellence (Policy 10-602) as well as his/her department's commitment to Operation Excellence.

A. (X) This position will involve performing procedures or other job-related tasks that has an inherent potential of contact with blood, body fluids, or other infectious materials as defined by OSHA to an individual's skin or mucous membranes will occur. The procedures or tasks could also result in spills or splashing on an individual's skin or mucous membranes. I understand that I must have a clear understanding of the proper use of protective attire and wherethis attire is maintained for use before performing any duties. I will discuss this with my supervisor and review a detailed list of these tasks found in my department's Policy and Procedure Manual.
