Pay Range: $55,000 – $63,000

Job Summary

The Assessor is responsible for conducting person-centered assessments to determine eligibility for programs and services. This role provides education and support to individuals and caregivers, ensures accurate documentation, and upholds compliance standards. The Assessor collaborates with internal teams and external partners to advocate for clients and connect them with appropriate resources.

Essential Functions

Conduct comprehensive, person-centered assessments to determine program and service eligibility.

Provide education and guidance to individuals, caregivers, and legal representatives regarding available resources.

Advocate for clients, with a focus on addressing health and safety concerns.

Report suspected abuse or safety issues in line with established policies and procedures.

Maintain accurate, timely documentation in electronic systems.

Qualifications

Licensed Social Worker (LSW) or Registered Nurse (RN) in good standing in Ohio.

Minimum of 1 year of experience in medical social work, home health, geriatrics, or a related field.

Experience With Electronic Documentation Preferred.

Strong knowledge of issues affecting the elderly population.

Proficient in Microsoft Word, Excel, Windows OS, data entry, and adaptable to new software.

Ability to work respectfully with individuals of diverse cultural, spiritual, racial, and ethnic backgrounds.

Reliable high-speed internet for secure remote system access.

Valid driver’s license and reliable transportation for travel within Butler, Clermont, Clinton, Hamilton, and Warren Counties.

Willingness to travel within Ohio for training.
