Job Details

What You Will Be Doing

Deliver compassionate, high-quality hospice care in patients’ homes or residential facilities

Manage care plans from admission to discharge, including medication and equipment coordination

Collaborate with interdisciplinary teams (Social Worker, Chaplain, PT, OT, etc.)

Present patient updates at team meetings and adjust care plans holistically

Educate patients and families, assess readiness to learn, and support discharge planning

Maintain compliance with healthcare standards and promote cost-effective care

Skills You Need

RN license in North Carolina

1+ year of experience with acute or chronically ill patients

Strong communication and organizational skills

Ability to work independently and prioritize care needs

Familiarity with interdisciplinary collaboration and holistic care planning
