Client: Large Payer HQ in Mid-West

Key Responsibilities:

• Conduct in-home MDS assessments in accordance with CMS and state home care regulatory requirements.
• Review patient health status, medications, and functional abilities during home visits.
• Collaborate with care teams, including physicians, therapists, and case managers, to support individualized care plans.
• Accurately document findings and submit assessments within required timelines.
• Educate patients and families on care plans, resources, and services.
• Ensure compliance with quality standards, patient safety, and documentation guidelines.

Qualifications:

• Active Registered Nurse (RN) license in the state of Massachusetts (in good standing).
• Minimum of 2 years of nursing experience (home health or community-based care preferred).
• Prior experience with MDS 3.0 assessments strongly preferred.
• Strong knowledge of CMS guidelines for home health care.
• Excellent assessment, documentation, and communication skills.
• Ability to travel to patient homes and work independently.

Preferred Experience:

• Certification as a Resident Assessment Coordinator (RAC-CT) or similar is a plus.
• Experience with home health EMR systems and electronic MDS platforms.
• Previous work with case management, discharge planning, or transitional care.
