Position: RN Hospice Hospital Liasion

Location: Essex County

Schedule: Monday to Friday during regular business hours, approxiametly 8 hour shift

We are currently looking for a Registered Nurse to join as a Hospice Hospital Liaison to join our team. The position would be responsible for assessing patients for hospice needs and transition of service and coordinating total care for hospice patients and their families

Responsibilities:

  • Assesses the needs of the patients and provides counseling to patients and their families and coordinates the transition to hospice to promote continuity of care
  • Establishes and maintains liaison with physicians, external and internal customers
  • Assists in managing and monitoring the quality and appropriateness of the services and programs
  • Conducts community outreach activities to improve community access to hospice care
  • Assists in developing and implementing a discharge program at one of our partner hospitals.
  • Adheres to agency infection control and safety policies
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position


Qualifications:

  • New Jersey Registered Nurse License
  • Bachelor’s degree in Nursing Preffered
  • Excellent clinical skills and experience in hospice nursing and community health nursing
  • Excellent verbal and written communication skills
  • Excellent customer service, organizational and interpersonal skills
  • Strong computer skills with proficiency in MS Office Suite


Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $80,000 to USD $135,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
