RN Case Manager

StartDate: 10/6/2025 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in an inpatient rehab setting

MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN license; BLS; 1+ year acute hospital case management experience

PREFERRED QUALIFICATIONS – Inpatient Rehab Experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7a-7p with every other weekend

SYSTEMS – Meditech

START DATE – 10/6/2025

This city is home to Candy Cane Park and also very close to the famous Lake Conroe where it got its name from. Corinna Kirsch, the famous art critic, is from Conroe. During the oil profits in the 1930’s, for a short period of time, Conroe boasted having many millionaires living in its city, and in 1955, Elvis Presley performed at a High School football field in Conroe.
