Position Description

We’re seeking experienced and compassionate Registered Nurses (RNs) to join our growing team, providing both high-acuity and low-acuity patient care across central and northern New Mexico. Whether you're looking for structured, full-shift assignments or flexible visit-based scheduling, this opportunity offers the autonomy and variety to match your lifestyle and professional goals.

We offer two types of assignments to match your skills and interests:

Low-Acuity:
Ideal for nurses seeking consistent hours with flexibility to manage their own schedules.
Care is provided in the comfort of the patient’s home.
Visit frequency varies by patient, from monthly, bi-weekly, or weekly.

High-Acuity:
Patients require extended care hours of 6 to 12 hours per day
Ideal for nurses looking for stable, full-shift work in a home setting
Includes more complex clinical care and monitoring
Opportunity to build strong, long-term relationships with patients and families

Requirements:
Active RN license in the state of New Mexico
Reliable transportation
Ability to work independently

Responsibilities:

  • Execute physician prescribed plans of care.
  • Complete compliant documentation of the care provided in the system of record.
  • Manage and administer prescribed medication, treatment, and therapies.
  • Conduct patient assessments, coordination of care.
  • Perform various activities associated with daily living.
  • Monitor vitals and GI intake and output.
  • Assess skin integrity and administer wound care.
  • Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
  • Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
  • Provide education and training to the patient, their family, and/or the Home Health Aide.
  • Work with patients to be done one-on-one in the patients home to provide for a comfortable environment.
  • Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
  • Perform other personal care services as necessary to meet the patient’s needs.
