Position Summary
Reporting directly to the Pediatric Transplant Program Director, the Clinical Coordinator III (RN III), has an overall focus on renal transplant patient care and coordination. The Clinical Coordinator III (RN) assists physicians with patient care, health education/promotion, electronic medical record (EMR) chart management, coordination of patient care and clinical protocols as needed. These duties include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, and patient education that can be in person, via our patient portal, or by phone. The Clinical Coordinator III (RN III) will work closely with multiple providers as well as other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values.

Responsibilities
- Assesses and collects data related to patient health status and inputs this medical information into the EMR as instructed by the physician.
- Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed.
- Administer immunizations and medications.
- Under limited supervision, executes nursing interventions by collaborating with the care team and following standards of care established by the unit/department.
- Supports on-going clinical functions of the service to provide compassionate patient-centered care.
- Handle telephone, My Chart and in-person triage (for symptom management) as well as patient education.
- Assist with management of urgent and emergent patient situations in conjunction with care team leadership.
- Coordinates multidisciplinary health care services and arranges diagnostic tests (i.e. lab, x-ray) as ordered by physicians and performs diagnostic testing, therapeutic, and interventional procedures as needed under physician supervision.
- Review and act on test results (lab, x-ray).
- Review and manage communications from internal and external “customers” inclusive of inbasket messages, updating immunizations, updating health maintenance items.
- Ensures infection control compliance within the practice.
- May float to other clinics on as needed for operations need.
- May precept new clinical staff including Medical Assistants, LPNs and more junior nurses.
- Other duties as assigned and appropriate to scope, education, and/or training.
- Pre-transplant evaluation of pediatric end-stage kidney disease patients, in coordination with the CUIMC kidney transplant team (this is a key part of this position).
- Coordinating evaluation process to transplant.
- Round with MD/transplant surgeons on new transplant patients who are admitted, if time permitted.
- Post-transplant follow-up for patients with transplant physicians in clinic and follow-up on outside labs.
- Review/teaching medications for transplant patients when in hospital and in clinic-Process prior authorizations for medications, DME and enteral formula for transplant patients.
- Triage patient phone calls from transplant patients (letters for school, communicate information to transplant teams on changes/medications).
- Ordering and following up on routine labs for transplant patients.
- Coordinate communication regarding renal patients with relevant providers, staff and parents.
- Data collection/coordination for Transplant studies (IROC and NAPTRCS).
- Provide back-up coverage for non-transplant patient care (such as triage and communicate lab results to families) when MA or RN is absent.
- Provide back-up coverage for 24-hour ambulatory BP monitor (when general renal RN is absent).
- Follow up on satisfaction survey.
- Develop Crash Cart check schedule.
- Monthly Renal Clinic Equipment Inventory.
- Other duties as assigned and appropriate to scope, education, and/or training.
- Requires a bachelor's degree in nursing or equivalent in education and experience.
- 4 years related experience. Clinical rotation experience associated with BSN degree programs will be applied toward related experience requirements.
- BLS certified.
- Current New York State RN License.
- Ability to work indoors in an environmentally controlled environment with a degree of dust, noise, odors, etc., without undue discomfort.
- Ability to perform sedentary work that primarily involves sitting or standing for long periods of time.
- Travel between clinical sites may be required depending on need.
- Duties performed routinely require exposure to blood, bodily fluid, and tissue. Incumbent in expected to perform and/or/assist in emergency care and/or first aid as needed.
- Bachelor’s degree in nursing.
- Minimum 1 year of inpatient or outpatient nursing.
- Experience with solid organ transplant experience is a plus.
- Candidate should have excellent interpersonal, oral and written communication skills.
- Bilingual Spanish is also a plus.
- Perform point of care testing.
- Pend orders for providers per protocol.
- Update the health record with historical items.
- Prep clinical forms (school, work) for provider signature
- Precept new employees.
