RN Case Manager

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in an ARU setting

MINIMUM REQUIRED QUALIFICATIONS – OK/compact RN license; BLS; 1+ year acute hospital case management experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – M-F full-time. Weekends on request.

SYSTEMS – Meditech

START DATE – ASAP
